1. Sign in to address http://www.jotform.com/
2. Well after entering there you will be provided tools of the form generator, before beginning to make you should first register in order to redirect the email later.
3. After completion form registers you please design your creations comments, there are provided many tools you can use just drag n drop it.
4. After you finish designing your current setup & share menu and you select a share after it forms a box will appear and you select the advanced option settings for pushing bloggers to get the code.
